The Patient Care Coordinator II is a member of a dynamic team focused on coordinating all aspects of patient care. The role supports Patient Care Coordinators with local and interdepartmental issue resolution, load-leveling, training, and reporting. The Patient Care Coordinator II is a member of the care service team and acts as a professional liaison for patients and families, care providers, as well as other FHCC departments. The role uses critical thinking skills to facilitate and operationalize complex scheduling requests through the heavy use of multiple computer systems. 

 

Please note that this is a Per Diem position.

Responsibilities

  • Performs the primary responsibilities of the Patient Care Coordinator I 
  • Assist in the management of day-to-day operations and serves as a point of contact to address patient needs 
  • Performs level-loading during planned/unplanned absences 
  • Ability to identify and triage complex issues appropriately 
  • Assists development of training materials in addition to training and mentoring new staff 
  • First point of escalation for problem solving issues, further escalates to PCC Lead/Supervisor as needed 
  • Demonstrates initiative as evidenced by advancing technical skills, sharing knowledge with others and taking on more responsibility 
  • Assists with more complex financial clearance process as necessary 
  • Communicates to the supervisor and fellow PCC's necessary instructions and feedback regarding department planning, improvements and processes. 

Qualifications

Required:  

  • High school graduate or equivalent
  • Ability to take applied knowledge and make sound decisions on behalf of the department 
  • Ability to review work to identify and eliminate errors and omissions 
  • Ability to work in a high pressure, deadline oriented, and complex health care environment 
  • Proficient in utilizing an electronic medical record system and/or an automated scheduling system. 
  • Ability to work independently, organize, and prioritize concurrent responsibilities and demand 
  • Ability to communicate verbally in person, on the phone, and in writing in a clear and concise manner 
  • Proficient in the use of word processing (MS Word), email, and data entry in a Windows-based environment 
  • Demonstrated customer service skills 

 

Preferred:  

  • Bachelor's degree (BA or BS) 
  • Minimum of 2 years of experience in a patient care setting preferred   
  • Experience with Epic or other scheduling and registration software 

The hourly pay range for this position is from $31.97 to $34.02, which includes the per diem differential, and pay offered will be based on experience and qualifications.
